The short answer
The zodiac has twelve signs because it divides the ecliptic into twelve equal 30-degree segments — a mathematical division, not a map of the constellations. Ophiuchus is a real constellation the Sun passes in front of for about eighteen days each year, and it has never been a zodiac sign in any tradition. The periodic 'thirteenth sign' story confuses constellations, which are irregular regions of sky, with signs, which are equal divisions of a circle.

They share names because the signs were named after the constellations that sat behind them roughly two thousand years ago. They have since drifted apart, which is a separate matter covered in sidereal vs Western astrology.
Asking why the zodiac has no Ophiuchus sign is like asking why a clock face has no thirteenth hour. The division was chosen; it is not a survey of what is up there.
Why twelve specifically
- It matches the lunar months. Roughly twelve lunations fit in a solar year, which is the oldest reason and a practical one for a calendar.
- It divides cleanly. Twelve is divisible by 2, 3, 4 and 6, which makes the aspect geometry work — oppositions, trines, squares and sextiles all land on sign boundaries.

- Thirteen would break all of it. A thirteenth sign gives a prime number of divisions, which destroys the aspect geometry and the element-modality structure entirely.
That third and fourth point are the substantive answer. The twelve-sign zodiac is a structure, and thirteen is not a small adjustment to it — it is a different object with none of the same properties.
Why the story keeps coming back
Every few years a version of 'NASA has changed the zodiac' circulates. NASA has never changed the zodiac and does not maintain one. What has usually happened is that an educational page explaining the difference between constellations and signs has been reported as a discovery.
The story is durable because it offers a very satisfying reveal — that your sign is wrong and you have been misled — and because most people have never been told that signs and constellations are different things. The reveal lands because the underlying distinction is genuinely not common knowledge.

This surprises people who assume sidereal astrology, being star-based, would include Ophiuchus.
It does not. Sidereal astrology also uses twelve equal 30-degree divisions — it simply anchors them to the fixed stars rather than the equinox. Both zodiacs are twelve equal segments; they differ only in where segment one begins.
So no established tradition uses thirteen signs. There are occasional modern attempts at a thirteen-sign system, and they have no historical basis and no following.
